Patient Care Coordinator
PCCs are assigned to specialized departments based on operational needs and are responsible for executing functions specific to their area.
The Patient Care Coordinator (PCC) plays a vital role in supporting patient care and clinic operations by acting as a liaison between patients, healthcare providers, attorneys, surgery centers, and external healthcare organizations. This position ensures seamless communication, efficient workflows, and a high standard of patient support across all interactions.
While all coordinators share the same commitment to patient care excellence and operational efficiency, daily responsibilities may vary depending on department assignment.
Key Responsibilities
Serve as the primary point of contact between patients, providers, attorneys, and third-party organizations
Ensure timely and accurate communication across all stakeholders
Support clinic operations through administrative and coordination tasks
Collaborate cross-functionally with internal teams to ensure continuity of care and successful patient outcomes
Maintain compliance with HIPAA regulations and internal policies
Department-Specific Responsibilities
1. Procedure Scheduling
Coordinate and schedule injections, procedures, and treatments via inbound and outbound calls
Communicate with patients, attorneys, and medical facilities to confirm appointments
Send confirmation details and instructions to patients
Notify Case Managers (CMs) once scheduling is completed
2. Recommendations Coordination
Prepare and distribute provider recommendations, treatment plans, and follow-up instructions
Review physicians’ schedules and generate reports and Rx forms
Send recommendations to attorneys and authorized parties via email
3. Liens Administration
Obtain, process, and maintain lien-related documentation
Communicate with patients and attorneys via phone and email to gather required documents
Ensure all intake forms, agreements, and records are properly completed and stored
4. Surgery Liens Coordination
Prepare and distribute surgery center lien agreements and related documentation
Communicate primarily via email with attorneys and authorized representatives
Escalate communication to phone calls when necessary
5. Medical Records Coordination
Process and send medical records to external providers such as therapists and specialists
Ensure all documentation is transmitted accurately via email or fax
Maintain strict compliance with HIPAA and clinic policies
6. Appointment Scheduling
Coordinate follow-up appointments and manage provider calendars
Ensure patients receive timely reminders and updates regarding upcoming visits
Additional Responsibilities
Perform other patient care and administrative duties as assigned
Adapt to changing operational needs, staffing requirements, and clinic priorities
Qualifications :
Strong communication skills (verbal and written)
Ability to manage multiple tasks and prioritize effectively
High attention to detail and organizational skills
Experience in healthcare, legal, or administrative roles is preferred
Ability to work collaboratively in a fast-paced environment
