About the Opportunity
We are seeking a dedicated and highly organized professional to join our team in a client-focused Case Manager role. In this key position, you will provide critical support to individuals and families by connecting them to essential services, coordinating care plans, and advocating for their needs throughout their journey with our organization.
Responsibilities:
- Conduct thorough client assessments to identify needs, strengths, goals, and service eligibility.
- Develop, implement, and monitor individualized service or care plans in collaboration with clients, family members, and partner agencies.
- Facilitate referrals, schedule appointments, and coordinate resources among healthcare, social service, housing, or community support providers.
- Advocate for client access to programs, entitlements, and support services, ensuring each person receives equitable and appropriate assistance.
- Maintain accurate and up-to-date documentation, case notes, and records in compliance with organizational and legal standards.
- Regularly evaluate client progress, update case plans, and adjust interventions as needed to achieve positive outcomes.
- Respond to client inquiries, provide education about available resources, and offer emotional support throughout the care process.
- Identify barriers or urgent needs impacting client wellbeing and take quick action to resolve issues or refer to appropriate services.
- Collaborate with internal teams and external stakeholders to ensure seamless coordination of care and information sharing.
- Participate in team meetings, case reviews, training sessions, and ongoing professional development opportunities.
Qualifications :
- Strong organizational skills and attention to detail, with the ability to manage complex caseloads and multiple priorities.
- Excellent verbal and written communication skills; able to build rapport with clients and collaborate across teams.
- Knowledge of local resources, community agencies, public benefits, and eligibility criteria is preferred.
- Familiarity with maintaining confidential client information and privacy regulations (e.g., HIPAA) is desirable.
- Proficiency in using electronic case management systems and office software (Word, Excel, email).
- Bilingual or multilingual language skills are valued but not required.
