Case Manager Assistant

Responsibilities:
- Assist case managers in collecting and documenting client information, intake forms, and consent documents with accuracy and confidentiality.
- Coordinate appointments, follow-ups, and service referrals for clients, ensuring clear communication with partner agencies and service providers.
- Respond to client inquiries and provide information or guidance related to available services, eligibility requirements, and program processes.
- Maintain and update client records, case notes, and case management databases according to established procedures and privacy guidelines.
- Prepare case summaries, correspondence, and reports as requested by the case management team or supervisors.
- Monitor deadlines for documentation and scheduled activities, ensuring timely completion and follow-through on action items.
- Identify urgent issues or barriers to client care and escalate appropriately for prompt resolution.
- Support the implementation of care plans and help track progress towards client goals, working collaboratively with multidisciplinary teams.
- Assist with general administrative duties to ensure efficient case management operations.
Qualifications:
- High school diploma or equivalent required; additional coursework or certification in social work, human services, or related fields is a plus.
- 1+ years of experience in an administrative, support, or client service role, preferably in a healthcare, social services, or nonprofit setting.
- Strong organizational skills with proven attention to detail, accuracy, and the ability to manage multiple priorities.
- Excellent verbal and written communication skills; able to collaborate effectively with clients, team members, and external partners of diverse backgrounds.
- Demonstrated empathy, patience, and professionalism in service delivery.
- Familiarity with maintaining confidential client records and knowledge of privacy regulations (such as HIPAA) is preferred.
- Proficiency in using office productivity tools (Word, Excel, email) and comfort learning case management or EHR software.
- Bilingual or multilingual abilities are a plus but not required.
